Should candidacy for Corona treatment be based on merit rather than necessity?

Sick workers at meat processing plants are probably at the top of many people's list of those who should be treated first, and rightfully so. First responders obviously deserve being at the top of the list also. Should this possible treatment be spread out in the most affected communities, or should the treatment be administered based on merit alone. Should those who are not considered "essential workers" be forced to pay for their treatment, or even denied treatment until it is known that the treatment can be produced enmasse.
